For departmental orders, complete a Supplies and Services Requisition form. Obtain the appropriate approvals/signatures and submit to Corporate Contracting. A purchase order number will be issued and your order will be placed.

Applicable sales tax and freight charges will be assessed on all purchases. All proceeds from hospital-specific merchandise sales benefit each hospital’s respective Auxiliary.

Return Policy: All stock items are returnable within 30 days if in new and saleable condition in original packaging. Original order number must be provided for proper credit to be given.
